How to Become a Client |
If you are living with HIV/AIDS and want to
become a client of The Momentum Project, or refer a client, please contact
Momentum’s intake staff to schedule an intake appointment at (212) 691-8100
extension #326, Monday - Friday from 9:00 AM to 5:00 PM.
Registering to become a Momentum client is
easy. Our intake specialists gather basic information (in either English or
Spanish) and have clients meet with a representative from each program. Your HIV status
will also be confirmed. The intake interview is followed by orientation to familiarize new clients with the full range of
services provided by The Momentum Project. Upon completion clients will be
served a nutritious meal and given pantry.
The Momentum Project does not discriminate

Confidentiality Laws regarding all medical diagnoses.
